Pick up address including an apartment number if applies, or facility name

Where is the patient going? (address or facility name and doctor's names  / suite number)

Reason for transport

Type of insurance coverage that the patient has

*Insurance Carriers and Medicare require the patient's social security number and policy number(s) for identification and to process 

the claim.

Southern Regional Emergency Medical Services accepts personal checks, Visa, Master Card, Discover and American Express.

Patients can make payments online, by mail to the remit address located on their invoice, or by phone contacting our main office location at 1 (229) 358-0001
